He was a lawyer in Richmond County, GA. He was appointed 1st Lieutenant, Company F, 1st Georgia Infantry (Regulars) on 1 February 1861. Appointed Captain and A. A. G., General McLaw's Staff, October 19, 1861. Appointed Major in Adjutant General's Department July 16, 1862, to rank from May 23, 1862.

On the Campaign

He was killed in action on 17 September 1862 at Sharpsburg. In his Report, General McLaws said of him

It is with sincere regret that I have to state my adjutant-general, Maj. T. S. Mcintosh, was killed, shot through the heart while carrying out one of my orders. The country has lost in him as brave and as gallant an officer and gentleman as any that survive him.

After the War

His family had his remains reinterred in Savannah in October 1867.
